1. Heating vs Cooling Technology

CoolSculpting and Emsculpt NEO both use advanced technology to reduce fat on the body.

The CoolSculpting technology utilizes cryolipolysis to freeze and destroy fat underneath the skin. Only the fat cells directly frozen by the CoolSculpt device get destroyed.

On the other hand, Emsculpt NEO uses radiofrequency (RF) energy to heat the cells and trigger cell death (apoptosis), essentially “melting” the fat away without pain. For added benefit, the heat radiates beyond the treatment applicator paddle by up to 4 to 6 inches, taking the effects beyond the areas directly treated for broader effects. Fat cells not directly targeted by Emsculpt NEO also shrink from the heat, helping melt more fat apart from where the applicator sits.

But Emsculpt NEO goes beyond just fat reduction. It also uses electromagnetic energy to stimulate muscle contractions and build muscle, all within the same treatment. As fat reduces and muscles grow, your body becomes more toned, rather than just slimmer.

2. Pain During the Treatment

Many patients do not experience pain with CoolSculpting. However, there have been anecdotal reports of extreme pain during the massage portion of the treatment and in the days after treatment. During treatment, patients often feel tugging, pulling, or intense cold before numbness sets in.

Most patients do not experience pain with Emsculpt NEO either. Treatment can feel like an intense workout, but because the device distributes lactic acid, you don’t feel the typical soreness that accompanies exercise. We even have patients describe the heat as very pleasant!

3. Downtime After Treatment

CoolSculpting has minimal downtime, but some patients report soreness, bruising, or numbness in treated areas.

The possibility of PAH, or paradoxical adipose hyperplasia, also exists—though rare.

PAH causes the treated area to expand, rather than reduce. This results in a firm, enlarged area of fat. Only about 1 in 3,000 cycles causes PAH, with other sources citing rates of up to 2% of treatments or 1 in 50 cases.

Emsculpt NEO has no downtime. Most patients return to normal activities immediately. Panniculitis, a rare side effect that causes inflammation in the fat under the skin, can occur, but it is typically due to not following treatment recommendations properly. Ample hydration is key to helping avoid this side effect.
